The Diabetes Educator is responsible to provide a full range of health promotion and disease prevention strategies on health issues related to diabetes.

Risk Factors of Diabetes

  • 40 Or Older (Down from 45 & older)
  • First Degree Relative with Diabetes
  • Dyslipidemia (Abnormal Blood Fat Levels)
  • Overweight
  • Abdominal Obesity
  • Aboriginal
  • Hypertension (High Blood Pressure)
  • Acanthosis Nigricans
  • Given birth to a baby over 4kg or 9 lb)
  • Schizophrenia
  • History of Gestational Diabetes (Diabetes Identified or Beginning during Pregnancy

Signs and Symptoms of Diabetes

  • Excessive Thirst
  • Excessive Urination
  • Unusual Tiredness
  • Blurred Vision
  • Headaches

About Our Service

We provide clients, their families and the community at large a basic understanding of diabetes as per Canadian Diabetes Association guidelines.
 
You will meet individually with the Diabetes Educator.  Consultations can be done in Ojibway or the English language.
  • You can participate in group education sessions and healing  circles or individual counseling.
  • Your care will be personally suited to you.
  • To arrange for a consultation visit, please contact your local health care team.
  • The Diabetes Educator will be in the communities twice a month.

Objectives of Mnaamodzawin

  • To provide culturally appropriate enriched seminars on the promotion of health and prevention of disease related to diabetes.
  • To provide support to individuals and families living with diabetes through healing circles.
  • To integrate services with UCCM First Nation Health Care Providers and external.
  • To provide basic education and awareness to individuals and families with diabetes to increase coping skills, reduce risk factors, and promote wellness and  optimal functioning.
  • To provide client centered care by screening, referrals and promotion of diabetes self-management.
