Home Care Program Nurse Manager &
Case Management
 
Manage and supervise all aspects of the Home Care Program.  Coordinating, staff, completing client assessments & referrals and ensuring all clients care plans are developed, implemented, monitored, and evaluated.  Reponsibilities include:
  • Provide leadership to the budget, planning, maintaing and reporting process required as per funding agreements
  • Coordinate and develop educational programs to assist staff with professional development to meet the health needs within the communities
  • Coordinate and case manage discharges from hospital and referring client appropriately to meet their needs to ensure client is supported and receiving an optimal level of care maximizing health outcomes
  • Assist with nursing home placement and advocacy for best possible care
  • Coordinates the development, implementation, monitoring and evaluation of the home care clinical nursing and personal care services
  • Assess clients and determines needs
  • In collaboration with the client and family, develop a plan of care to address needs
  • Coordinate and monitor plan of care until the clients goals are met
  • Determines eligibility, provides ongoing assessment and planning, linking, coordination and authorization of services and resources for clients reffered to Mnaamodzawin Health Services Home Care Program
  • Equipment loans, such as: beds, shower chairs, walkers, commodes, etc.
