Personal Support Worker Coordinators
Coordinates and provides care contributing to the quality of life of individuals who live at home by promoting their independence, well-being, mobility, comfort and safety, so that they remain and participate in their community.
 
Programs and services include:
  • Social and Congregate Dining
  • Generic transportation
  • Personal care and Homemaking
  • Meals on Wheels
  • Day services & activities
  • Friendly visits & security checks
  • Collaborates with other health care providers and Registered Nurses
  • Adult program to enjoy social and recreational activities

 

Personal Support Worker
This program provides health services with the intent to promote independent living.  Services may include:

  • Personal care (i.e. bathing, assist with daily living)
  • Meal preparation according to Care Plan
  • Generic transportation (Escort when physical assistance is required)
  • Respite care; to give family caregivers short periods of relief
  • Day services
  • Friendly visiting
  • Palliative Care
  • Deligated Acts
