Our Mission

The Home Care Program is committed to ensuring health services are equitable, accessible, and culturally appropriate. We strive to preserve and enhance each individual’s ability to live independently at home by providing safe, effective care and support.

Program Goals

  • Collaborate with other service providers to improve community care and eliminate service gaps or duplication.
  • Support clients to live as independently as possible within their communities, while preserving and strengthening family and community support.
  • Deliver services efficiently and responsibly, demonstrating accountability for the use of public funds.

Personal Support Services

  • Friendly home visits and security checks
  • Assistance with activities of daily living
  • Pressure injury prevention
  • Equipment loans (wheelchairs, hospital beds, walkers, shower chairs, commodes, etc.)
  • Meals on Wheels
  • General transportation support
  • Respite care
  • Assisted Living Program

Nursing Services

  • Care coordination
  • Foot care/chiropody clinic referrals
  • Health risk assessments
  • Accompaniment to medical appointments
  • End-of-life care and palliative support
  • Referrals to mental health services, dietitians, and traditional healers
  • Medication administration and monitoring
  • PICC line maintenance
  • Intravenous therapy maintenance
  • Blood draws
  • Vital sign monitoring
  • Occupational and physiotherapy referrals
  • Hospital discharge planning
  • Wound care management
  • Long-term care referrals
  • Individualized care plans
